I hereby apply as a volunteer with The Smith Center for the Performing Arts. I certify that, to the best of my knowledge, all answers to questions on this application are true, and I understand and agree that any intentional misrepresentation of facts contained in this application may disqualify me for a voluntary opportunity. I understand that I will not be paid for my services as a volunteer and that filling out this application does not guarantee acceptance into the volunteer program. Due to the uniqueness of our operations and activities, volunteers will be needed at a variety of times during our business hours, including weekdays, weekends, nights, and holidays. Volunteers agree to be scheduled for assignments according to the needs of the center. The Smith Center candidates who are selected for our volunteer program are also requested to complete the following steps: Meet-and-greet or recruitment interview, background check, orientation/training, and agree to sign the Volunteer Participation Pledge form. *
